About Point-of-Service Discounts...

The Point-of-Service Discount Card your patient has presented to you entitles the Cardholder and eligible dependents to receive point-of-service discounts for services performed by Providers who participate in the Galaxy Health Network Preferred Provider Organization (PPO). These discounts should be applied in accordance with the Provider's fee schedule agreement with Galaxy Health Network.

Unlike providing discounts to participating group and individual medical insurance plans, point-of-service discount plans afford many advantages to participating Providers. Among them are:

• The Cardholder/Patient pays for services when they are performed
• There are no claim forms to complete
• There is no waiting for payment from the insurer
• Private patient information remains in the Provider's office
